Our staff of professionally trained Case Managers perform assessments to assist individuals and their families in planning and care. Case Managers address a variety of needs through our programs and services while providing ongoing support.

Our case managers connect with individuals and families by providing professional assessments and ongoing monitoring to ensure that the older adult is receiving proper care.

Each year nearly 2,000 hours of case management service are provided to individuals and their families.
